The transaction was announced by Compass' sell-side adviser, Polaris Healthcare Partners, in a release dated November 2. Financial terms were not disclosed.

Compass founders Dr. Scott Ballard and Dr. David Ballard will remain in leadership roles and become shareholders in Elevate. Compass' partner dentists will continue to lead their practices as Elevate partners.

This is a recapitalization rather than a simple change of ownership. The operating model preserves local doctor leadership while adding the capital and centralized support of a larger dental partnership organization.

Why this transaction matters

Compass gives Elevate an established operating cluster in a new state rather than a collection of isolated affiliations. The group was built over more than two decades and brings seven locations, 91 operatories, existing management infrastructure and local market density.

With Compass, Elevate now supports practices across Colorado, Washington, Arizona, Kansas and Texas.

That geographic expansion matters because regional density can improve recruiting, purchasing, marketing, specialty referrals and field support. Avized's analysis of Heartland Dental's 33-practice Foundation Dental transaction showed the same strategic logic at a larger scale: groups increasingly value established regional platforms that can be integrated as operating clusters.

The Compass deal also fits the broader ownership shift described in Avized's review of TUSK's Q4 2026 dental consolidation report. More dentists are entering group practice, more established owners are approaching succession decisions, and private-equity-backed platforms continue to seek durable practice-level earnings.

The rollover-equity structure is the key signal

The most important disclosed term is not financial. It is that Compass' founders are staying in leadership and becoming shareholders in Elevate.

That structure aligns the sellers with the future performance of the combined organization. It also reduces one of the central risks in dental M&A: losing the doctors, leadership and culture that created the acquired group's value.

For other multi-location practice owners, this illustrates the range of outcomes between remaining fully independent and selling outright. A recapitalization can provide liquidity and operating support while preserving clinical leadership and continuing equity exposure.

The tradeoff is execution. The value of rollover equity depends on the larger platform's future growth, governance, leverage and eventual liquidity event. Those details were not disclosed.

Integration will determine the outcome

A seven-location group is large enough to have its own workflows, systems, compensation structures and local culture. Elevate will need to decide what should be standardized and what should remain local.

Technology will be one of the clearest tests. Growing dental groups need common visibility across practice management, revenue cycle, patient communications, analytics and procurement without disrupting acquired offices. Avized's technology-stack framework for 10- to 100-location DSOs explains why integration architecture—not simply software selection—becomes more important as groups scale through acquisition.

The operating question is whether Elevate can create shared infrastructure while preserving the autonomy that attracted Compass' dentists to the partnership model.

What to watch next

Doctor retention. The founders' continuing leadership and equity are positive signals, but long-term performance will depend on retaining partner dentists and practice-level teams.

Texas expansion. Compass creates a North Texas base that could support follow-on affiliations or de novo growth in adjacent markets.

System standardization. Watch whether Elevate moves Compass onto shared technology and support workflows or maintains a more federated operating model.

Specialty development. A 91-operatory cluster can support internal referral networks and specialty expansion if patient volume and geography align.

Additional regional groups. The transaction may indicate that Elevate is willing to add established multi-site organizations, not only individual practices.
